Work Permit and RMV Information
Work Permit Information
1) The student must complete the Promise of Employment Form for 14 Through 17 Year-Olds.

2) Bring this form to your employer. Employer must complete the Promise of Employment section.
3) For 14 and 15 year olds only (16 and 17 year olds may skip this step): Physician's Certificate of Health section must be completed by your physician.
4) Parent/Guardian must sign the Parent/Guardian Permission section. The student must then sign the bottom of this form - “Signature of Minor.”
5) Bring the completed Promise of Employment and a copy of your birth certificate or passport to Ms. Robertson in the Guidance Office to fill out the work permit.
If it is dropped off in the morning, it will be ready by the end of the school day to sign and pick up.When school is not in session, you will need to contact Ms. Emerick at the Superintendent's Office to set up an appointment to process your work permit. 508-278-8648 x2002
RMV Information
When obtaining your driving permit through the RMV, you will need to provide a proof of residency. You can request a transcript as that proof. Please follow the instructions below to request it. Please request at least a week in advance of your RMV appointment.
To request your transcript for a proof of residency, use this form. Click on "this form" link to bring you to the google form request. Once the request is received, it will be processed and you will get an email when it is ready to pick up.

Click on Departments, School Counseling and Transcript Requests. Scroll down to Transcript Requests for the RMV and click on "this form" and fill out the google form.
