Uxbridge High School Alumni/Former students who need to request a copy of their high school transcript, please complete the transcript request form using this link.  

Emailed transcripts are not official. Please download and complete the form and mail or drop off to Uxbridge High School, Guidance Office, 300 Quaker Highway, Uxbridge, MA 01569.  This form can also be emailed to Ms. Robertson  jrobertson@uxbridge.k12.ma.us

There is no charge for transcripts. We have no affiliation with Need My Transcript website

When the Guidance Office is closed for the summer or school vacations, transcripts requests will be processed when the Guidance Secretary is in the office.  Please see Guidance Office Hours for the days Ms. Robertson will be available during the summer.

The RMV recently changed its procedure, in that now transcripts are required as a proof or residency for license applications/road test.  To request a transcript, use this form. 

Please request at least a week in advance of your RMV appointment.
